Job Description

POSITION DESCRIPTION: Clinical position to monitor Complex Chronic Disease Patients in a case management role via tele-monitoring. Involves telephonic assessment of a patient’s reported symptoms, disease process, and biometric status, triage of critical situations, and education on their condition(s), co-morbidities, medications and lifestyle modifications. Duties also include timely communication of appropriate medical information to the patient’s provider or case manager as needed.
Patient Advocacy Nurse Specialist reports directly to the PASS Clinical Services Supervisor.

POSITION RESPONSIBILITIES:
  • Daily monitoring of patients in the Patient Advocacy and Support Service Center which includes:
  • Addressing all Biometric alerts daily. Prioritizes calls by acuity. Call patients to verify reported symptoms & biometrics. Follow-up with providers and case managers as required per protocol.
  • Outreach calls with patients to acquire past medical history with an introduction to our program.
  • Scheduled outreach calls, after a hospitalization, to educate the patient on their disease process, medication reconciliation, ensure provider follow-up, and establish any barriers to care.
  • Documenting nursing interventions taken during telephonic outreach/assessment.
  • Updating patient records based on information received from providers/case managers.
  • Reviewing and analyzing generated Trend Reports within department software prior to telephonic assessment.
  • Occasional preparation and review of monthly Progress/Status Reports with the client on conference calls.
  • Customer Service as it relates to basic device troubleshooting.
  • Procurement of additional state licensure as assigned by PASS Clinical Services Manager/Supervisor.
  • Will be required to travel to Medtronic offices for clinical training as assigned.
  • Other projects as required and assigned by the PASS Clinical Services Manager/Supervisor.
