Description
The Community Based Instructor provides instruction and supervision to individuals participating in the Community Services Program, including Community Habilitation. The Community Based Instructor works with individuals in their own homes and in the community to increase their independence in activities of daily living.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

•Provides direct instruction to Community Based Services (Community Habilitation) participants to increase their independence in the areas of daily living, including self-care/personal hygiene, food skills, community integration and leisure recreation activities.
•Communicates medical concerns to Service Coordinator.
•Assists in developing Community Habilitation Plan with individual/treatment team.
•Maintains frequent contact with Community Program Manager or Senior Community Manager and communicates as needed with Service Coordinators/Community Service Providers and family/guardians.
•Participates in general staff meetings, in-service training and other pertinent agency meetings.
•Assumes responsibility for Community Habilitation program and assists individuals during assigned hours which may include days, evenings and weekends.
•Safely transports individuals to and from activities and appointments.
•Completes daily documentation, Quality Assurance Checklists, individuals files and other paperwork as necessary
•Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).
•Complies with all agency policies and procedures.
•Other duties as assigned.


Minimum Requirements
•18 years of age.
•Valid NYS Driver’s License
•Ability to transport individuals in personal vehicle that meets agency requirements.
•Ability to read and write.
•Ability to handle multiple tasks simultaneously.
•Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, squatting, kneeling and climbing.
•Lifting requirement of 35 lbs.
•Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention Core Training certification (SCIP-R) as per regulatory guidelines.
•CPR certification required.
•Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan.
•Ability to move, reposition and place individuals in pieces of equipment.
•Therap and billing knowledge preferred.
